Hello,
I am interested in moving my companies incident report forms to the web. I would like for the users to be able to fill out the forms which populate the PDF and then have the PDF be sent via email. I tried to do a test form but it can only map to a text field called, I’m not getting the visual field. I’m assuming this is due to my PDF, which program should I use to make a working PDF? As for sending via email, can I create a check-box that will change the recipients of the email?

To create PDF Forms you should use Adobe Acrobat (any version except Reader) or PDFEscape.com.

Yes, you can change the eEmail address of the recipient by using the field id of your check-box in the Formidable Forms -> Settings -> Form Actions -> To: text box.
