I have been using the plug in successfully on two sites for several years to fill in patient medical histories for dentists. I had to update their PDF forms, I uploaded the new form, made changes in formidable and remapped all the fields to the Formidable Form. None of the checkboxes are checked anymore when the form is submitted.

LindaAcrobat DC Pro does have the “Export Value”. Our version has it in the same location, and we are unable to find any version without the setting.
If you are still unable to locate the setting, you can upload the PDF form to PDFEscape.com to access the settings.
We would need to see the PDF form in order to specify the issue. Use this form to upload the PDF and/or provide access to your site for further review.
