On the form, I’ve named the field and added several option descriptions.
On the pdf, I’ve given each checkbox a unique name and enter in the value the option description fro the form.
In the filed map, I’ve mapped the field name to each corresponding form field name for each option and selected “no formatting”.
I fill out the form and check all boxes as a test.
When I run the merge, all checkboxes are blank.
What next?

We apologize for the delay, in the meantime, a work-around would be to create and map one checkbox field for each PDF field, however we expect to resolve this issue quickly.
