Hello,
We are working on a project of one of our client where the requirement is generating PDF and sending it via email. It will be a form with several radio buttons, checkboxes and text boxes. Around 50 form fields will be there. here we need to make sure that the generated PDF will be same as predefined PDf file which we will provide you. Please let us know if this can be done by your plugin. Its a urgent requirement.

Yes, if we understand correctly, where you have numbers is where a PDF form field will go, could be a checkbox or a text box, at first glance we would suggest a checkbox depending on how you want to input the data into the web form.
