September 21, 2016 at 5:19 pm #9835adminKeymasterWe show the key for your order #9832 is currently active and in use (22 pdf’s created) on https://shouldertoshoulder.com/
Please confirm the site above and that you are still receiving the error.
We remain at your service.
September 22, 2016 at 8:04 am #9840SladeParticipantI can confirm that https://shouldertoshoulder.com is still getting the error.
September 22, 2016 at 9:00 am #9841adminKeymasterOdd, our system shows the key as actively creating PDF’s. We have not see an error like this before, if it is possible for us to login to your installation to troubleshoot the issue, please complete this form.
If not possible, we will try to troubleshoot with the following;
– Has PRO2PDF successfully produced any PDFs on the site? If yes, please explain.
– Verify the PDF form is not a LiveCycle Form and also does not contain Adobe “Signature” form fields.
– Provide screenshots of; “Under the Hood” tab and the “Export” Tab with the Form, Dataset, and Field Map Layout selected and include all the mapped fields.We remain at your service.

September 22, 2016 at 11:05 am #9845adminKeymasterForm received, however, credentials were invalid. Please check and advise, as we remain at your service.
September 22, 2016 at 11:20 am #9846SladeParticipantI recreated the user and double checked the login. I have resubmitted to your form with updated credentials. It should be working.
September 22, 2016 at 2:07 pm #9848adminKeymasterStill not working, please check credentials again and let us know…
September 22, 2016 at 2:21 pm #9849SladeParticipantI used those credentials myself and they worked. I passed them off to two other people who could also get them to work. Those credentials are working.
September 22, 2016 at 2:43 pm #9850adminKeymasterUsing copy/paste to post the credentials we are receiving a “ERROR: Invalid username. Lost your password?” message from two different IP’s and three browsers. I’m afraid the credentials are not allowing us access to the site. Please enter our email address into the account so that we may try changing the password. support AT formidablepro2pdf DOT com.

September 23, 2016 at 8:22 am #9869adminKeymasterThank you, however, the Lost Password page is missing from the site (404 Error) https://shouldertoshoulder.com/lost-password/
We are at a loss. Would you try creating a new account, but this time enter our email address and select “send password to user”?

September 23, 2016 at 8:36 am #9870SladeParticipantThis has been done
September 23, 2016 at 11:24 am #9872adminKeymasterThank you. We were able to access the site.
It appears the Health_History…PDF has not been turned into a form using Adobe Acrobat or other PDF form creation software which is required for the plugin to ‘merge’ data to the form fields. We converted the PDF into a form with one field and all works as expected. Here is an example of the merged document.
If you need assistance with turning the pdf into a PDF Form, please consider this Fiverr gig to complete the task for you.
We’ll check into why the error message was not accurate and submit an update soon to fix message to WordPress.org.

September 23, 2016 at 12:22 pm #9874SladeParticipantI am still having the same problem. I created my PDF in adobe indesign. I then opened it in adobe acrobat and created the input fields and saved that pdf again. I can map the fields to the fields i created in formidable which is something i cannot do with your pdf, not sure why that is. The problem I am having is the attached pdf in emails sent after the form is submitted. Even with your test pdf the attached document is an error.txt file with the following message.
There was an error generating the PDF file. This activation code is not valid.
I need the information submitted on the form to be attached in the email to the admin when the submission is made. I was under the assumption that this plugin could do that. Does it matter if i save the pdf as a high quality pdf vs. a reduced file size pdf or anything like that?
September 23, 2016 at 12:38 pm #9875adminKeymasterWe created a PDF with only one PDF form field, that’s why you can not map the other fields, there is only one.
Regarding the email error, it’s being caused by the non-working field map layouts.
Open all unused, test, and failed Field Map Layout and change “Attach file to Email notifications” to “NO” then click “Save Field Map”.
Or – – it’s better to delete all unused, test, and failed “Field Map Layouts” that are associated with the Formidable Form.
To delete all unused, test, and failed “Field Map Layouts”;
1. Select the “Select the form to export data from:”
2. Select the “Field Map to use:”
3. Scroll to the bottom of the field mappings and click “DELETE ENTIRE FIELD MAP”, and confirm the action.
4. Repeat until all unused, test, and failed “Field Map Layouts” are deleted.Then test and let us know the outcome while we remain at your service.
September 23, 2016 at 1:07 pm #9876SladeParticipantI have fixed my error. Thank you for your help and patience.
September 23, 2016 at 2:17 pm #9879adminKeymasterGreat news! You’re welcome, it was our pleasure.
