Can't Map Radio Buttons

Hi

I’ve got 9 yes/no radio buttons. On the map preview everything maps correctly. However, on submit only 1 of the 9 outputs correctly. In the confirmation email entries- everything is correct, in the outputted PDF it is not.
I’ve spent hours trying to work this out and many different combinations. If you look on the site you may not see the 9 combinations as I’m trying to get this finished.
Any help will put me out of my misery.

– Make sure the field names do not have any special characters.
– Make sure the radio group field names match and the field values are exact (they are case and space sensitive)
