If all else fails, use the checkbox field’s “Use separate values” option then give the checkbox options the values of 1,2,3,4,5… and change the PDF checkbox export values to the same.

There are two ‘special characters’ that were causing issues….
– Exclamation point in; “Staff Orientation – May 27-June 3rd; All staff required to attend!”
– Plus sign in; “Google +”

The image field was not found in the field map layout, it worked once it was mapped.

We duplicated the field map layout and converted all values back to before “Use separate value” so the prior entries will still work. Except for the two option values listed above.
