I’m unable to map check marks to appropriate selection in the form. Can you please help?
All PDF Checkboxes for one field should have the same name, and make the “Value” for each match the “Value” for each of your Formidable Checkboxes.
If you have done the above, please email your pdf form and a link to the Formidable Form to support (at) formidablepro2pdf DOT, com so we can provide specific instructions relating to your form fields.
Oops! I apologize, I forgot to mention that for “Multi-Select Checkboxes” you must name each of the PDF Checkboxes differently.
So for your PDF form, change “check up to five” to different names for each checkbox, then map the one Formidable Checkbox field to each of the PDF Checkbox fields, the plugin will then do the rest.
