I have a client that is a dental practice. They would like to be able to have their patient intake forms online so the new patients can input the information and the resultant entries downloadable in PDF form very similar to their current forms.

I want to use Formidable Forms for the online intake process. I’ve been told that this plugin will allow me to convert the input into a PDF. Do you offer consultation assistance for something like this? What would your rate be?
