Good day,
What an awesome and amazing plugin. Just stumbled across it and will definitely be using it for other projects.I don’t know if I’m doing something incorrect, however just after choosing my form and pdf, at the mapping input point. It seems to give me all my PDF input options, however the form options are limited (ie. I only see text inputs, no checkboxes, radio buttons, etc) However the demo has these available
I even tried editing the demo form to my requirements, however then the input options on this became limited to text only. I’m at a loss and would really appreciate some assistance 🙏

Did you check out our newest plugin called E2Pdf? It’s absolutely amazing and getting better all the time! We are working on a function that will automatically create web forms based on uploaded PDF forms in most or all of our extensions, Formidable, Forminator, Caldera, Divi, and soon Gravity, Contact Form 7 and Ninja.

The check/radio fields are there, as a single entry – you will NOT see each of field’s the options…
Here’s a downloadable example.
If you need more specific assistance let us know, it would be helpful if you could provide a link to the form or screenshots of the form and the Field Map Layout with the form and layout selected, use this form to provide the info.
