PDF stopped attaching

Out of the blue on Wednesday, July 7, our forms stopped having PDFs attached to the submission emails. Our IT department says all other emails are coming through with attachments so it’s not related to our email server. I made no changes to the forms and our account is paid for. I have no idea what’s causing these forms to not have the PDF attachment.

What could be going wrong here?

Let’s troubleshoot the field map layout.

Confirm that the submission emails are received, and there is no .txt file attached?
Also, provide the following screenshots;
1. DASHBOARD → FORMIDABLE PRO2PDF → EXPORT → select Form & Field Map → SCREENSHOT all settings and the first field mapping.

2. DASHBOARD → FORMIDABLE → select form settings → ACTIONS & NOTIFICATIONS → SCREENSHOT entire page
