From the plugin’s Field Map Layout screen
. Select the Form and the associated Field Map Layout
. Scroll down the layout and check the “Attach file to Email notifications” checkbox
. Check the “Email Notification” checkbox that will appear after the above box is checked
. Save the Field Map Layout

That’s it, all form submissions will receive the merged pdf based on the field map layout settings.
