Hello,
I have a similar problem.
Most of the fields are mapping properly. However I can’t seem to make the
checkboxes to get “checked” (with an “X”) on the PDF…
Also, the address field doesn’t display any part of the address; I do not manage to find how to make it display properly the data filled into the form.

AdrienadminKeymaster1. Use PDF Checkbox fields for Formidable Checkbox fields and use PDF Radio Button fields for Formidable Radio Buttons.
2. Use copy/paste to insert Formidable checkbox field values into the PDF checkbox field “Export Value” and Formidable radio button options into the PDF radio button “Option Value”.
3. See this example for Acrobat and this example for PDFEscape.If you need further assistance, use this form to upload your PDF form, provide a link to the Formidable form, and let us know which PDF forms editor you are using so we may be specific with our assistance.

I tried the described steps above, but still don’t manage to get the check-boxes to show up on the filled in PDF…
Right now I am trying to get by example the check-box which says “prepaid?”, as well as the ones above which asks:
“Je désire respecter l’échéance du contrat de mon opérateur actuel.”
and
“Je désire quitter mon opérateur actuel sans respecter la durée minimale du contrat courant,
et j’accepte de m’acquitter des frais éventuels que cette résiliation anticipée pourrait provoquer.”Thanks again for your help.
