Are you able to create a successful merge when using the EXPORT TO PDF button on the EXPORT tab after selecting the Form and Dataset?
If yes, then upload a screenshot of the EXPORT tab after selecting the Form and Dataset to this form and a screenshot of the shortcode in the form’s On Submit Message Box, or other location.
If no, then upload a screenshot of the EXPORT tab after selecting the Form and Dataset to this form be sure to include at least the first row of mapped fields or provide login credentials if possible for us to review the installation.
