We had to update to Formidable Pro. Followed steps they provided. Forms are there.
However when we try to submit application on http://eacougar.com/pre-registration/ it does prompt the user to download but email I get does not include the attached PDF.Open the plugin’s field map layout for the pre-registration form and see if the “Email Notification” is checked, if not, check it and test. If it is checked, then un-check it – save – reopen and recheck it – save – and test.
