Based on the description of your issue it sounds like a question for the great team at Formidable Forms. Once the email is sent and received the PDF should be attached as expected.
Take a look at this support thread before entering a support ticket with Formidable, it may solve your issue.

The PDF form MUST be uploaded to the PRO2PDF plugin from the EXPORT tab, there are no other options, it must work. If the PDF form is too large (more than 4mb) you’ll need to shrink the form.

Try this…
1) Check for any other field map layouts using the same form and make sure they too have the same field selected as the dataset name.
…. if that doesn’t work…
2) Try deleting the mapping for the field that you want to use as the dataset name, save the field map layout, then add the field mapping back to the layout and select it as the dataset name.

Name each PDF Radio option in a group the same, but you must change each PDF “Radio Button Choice” (also known as “Export Value” for checkboxes) exactly the same as their corresponding Formidable Radio Button Option value.

Disable the email attachment from the field map layout by selecting “No” for the “Attach file to Email notifications” option.
Do not use the shortcode in the On Submit Message Box or the email body if you do not want the user to download the PDF.
PDFs are generated ‘on-demand’ and not ‘saved’ as PDFs on the web server. Create a Formidable View for admin users only to view/download the PDFs on-demand.

A dataset is all the data that is entered into a web form when the form’s submit button is clicked.
If you have one PDF (or paper) form with 6 pages, then you would only need one web form and one fillable PDF form.

. On the SETTINGS tab, select the option to “Enable automated layout creation for new field maps”.
. Name PDF fields EXACTLY the same as the corresponding Formidable Form field name. If there are more than one field with the same name then name the PDF field the same as the Formidable Form field ‘key’ which will allow the plugin to auto-create the field map layout.Let us know if you require further assistance as we remain at your service
Email subject lines are not controlled or effected by the PRO2PDF plugin, you’ll need to contact the outstanding support team at Formidable Forms.

PRO2PDF does not add form fields to PDF files, it simply merges data from Formidable Form Fields to existing PDF Form Fields. You must add PDF form fields using Adobe Acrobat or another Adobe Form app such as PDFEscape.com.
