We have found a work-around but have not found the cause of the issue. Our senior developer is not available until Monday, so we have added an additional site to your license and all is working as expected. Our senior developer will need to look at the server on Monday to determine the cause and final solution.

One solution would be to use add a hidden “Text Calculation Field” to your form to add the various fields to just one field, then map the PDF to the hidden field. Let us know if the solution will work for this situation.

For Korean text in PDFs, change the “Language support” setting for each field map layout to Unicode.
For exporting CSV files, submit a support request with the great support team at Formidable Forms.

Use the “PDF password” setting on the Field Map Layout to avoid the URL method.

Add a hidden field to the form and then reference the field ID in the shortcode (password=[123] where 123 is a hidden field in your form)
Also, looks like there is a duplicate parameter and a space missing before ‘label’ in your shortcode… (layout=”1006″label=0)

The server must have internet access to merge signatures. Local or intranet installs will not merge signatures or images.

Confirm the PDF file name does not contain any special characters, commas, or periods and upload to PRO2PDF again. Also, make sure the PDF is not more than 6MB.
