Not sure, but usually a blank screen is a CSS issue with another plugin or the theme. Revolution Slider recently caused a similar issue that was fixed by updating the slider plugin.
If possible, please complete this form to have us take a look at the installation, or try disabling other plugins to see if you can find the cause. If it is not a plugin, then try changing the theme, and let us know what you find so that we may try to duplicate and fix the issue.

Have you entered data into the Formidable Form? If not, you must submit data at least once before you can create a field map layout.

After one year the Pro “Internet” Version will expire if not renewed, however, the free version will still work (1-Site | 1-Form). Subscriptions can be renewed at any time, or even purchased for up to 5 years.

1. Upload two PDF forms to the plugin, with and without the watermark embedded into the form, then map to one, duplicate the map and change the layout to point to the other, then make the LayoutID shortcode parameter dynamic based on the “Draft” or “Final” status of the form.
2. If you have a form condition that changes the form from Draft to Final, such as a radio button, you can map the button’s value to map a PDF Form text field with “watermark like” font settings and is set behind other fields in the PDF form. Using Formidable’s “Use different value”, with “DRAFT” as one value, and a blank as the second value.

Yes, the plugin has been tested and is verified to work with the most recent version of Formidable Forms as recent as 5 mins ago, based on this support request.

Awe, unfortunately emailing is currently limited to only one dataset, however, you can place the shortcode in the body of the email notification to provide a download link in the email to the pdf merged with both datasets.
Remove the shortcode from the On Submit message box if you don’t want the download link after form submit.
From the plugin’s Field Map Layout screen
. Select the Form and the associated Field Map Layout
. Scroll down the layout and check the “Attach file to Email notifications” checkbox
. Check the “Email Notification” checkbox that will appear after the above box is checked
. Save the Field Map LayoutThat’s it, all form submissions will receive the merged pdf based on the field map layout settings.

You are TOTALLY AWESOME! It was all setup correctly, the only reason it didn’t work is because your shortcode had field [2663] for dataset2, but the hidden field of the embedded form is [2662]. The main form does not require a hidden field ([2663]).
After double-checking everything, all I did was change 2663 to 2662 and everything works as expected! We will update the documentation with more detailed instructions (with images) to explain the hidden field setting.

There is a conflict between the Revolution Slider and the Theme that may be solved by updating the Revolution Slider from 4.6 to the recent version 5.1.6 (For fatal error see the debug.log). We are currently using version 5.1.6 with no conflict, however, with a different theme.
The same issue was reported here, and the author of RevSlider says it comes from the theme…

Hi! It’s a bit tricky, complete this form and provide the form names and field map layout names and we’ll make sure it works and tell you what needed to be changed. Then, we’ll update our documentation to make it easier to accomplish.
