adminKeymasterYou must submit data to the Formidable Form at least once before you can setup the field map layout.
Once data has been submitted, you’ll need to pick the Form and Layout, (the most recent dataset will populate the dataset dropdown), then you’ll be able to create (and test) the merge with your field map layout.

The switch form menu is a Formidable Forms dropdown, not our third-party add-on plugin. We are using auto-size for all of our form drop-down lists.

adminKeymasterDouble check the “Format” dropdown for the mapped fields in the Field Map Layout, should be set to “No Formatting”, if it is… then to further assist please provide a screenshot of the Field Map Layout screen with the Form, Dataset and Layout selected and to include the mapped fields. If we don’t see the issue there, we may need to look at the PDF form and possibly the Formidable Form.

adminKeymasterWe understand your need to change the PDF page output based on the form’s submit data, we too had the same need.
While the plugin will not change the structure of the PDF, and there are no intentions to add any PDF structure changing ability to the plugin, you can make the LayoutID variable and dynamic based on the form’s data, which allows you to have multiple Field Map Layouts from one Formidable Form to many PDF forms.
For your particular scenario we would:
. Create a master PDF Form (Form-with-images.pdf) which includes all possible fields.
. Map the master PDF to the Formidable Form (Form with images).
. Edit the master PDF Form to remove image page(s) and SaveAs Form-without-images.pdf
. Create a Duplicate of the “Form-with-images” field map layout by selecting the layout, then click Duplicate at the bottom of the map.
. Upload the new “Form-without-images.pdf” to the Field Map Designer and save it to the new layout.
. Delete the unneeded fields from the new layout.Now, you’ll need to make the LayoutID dynamic…
Use the Formidable Shortcode Conditionals such as IF STATEMENTS directly in your shortcode, or to decide which shortcode to use or not to use…Please let us know if you need more help with this issue as we remain at your service.

adminKeymasterHi, thank you for your patients and for submitting this issue on the support forum, as this is an issue we have not seen before…
The font type you used “Kozuka Mincho Pr6N R” is not happy with being used in a PDF form, I’ve changed the font to many other types and all works as expected… (other issues with this font are found on Adobe’s site…)
SOLUTION: Change the PDF form field fonts to nearly anything else…
When updating the PDF form simply edit the PDF on your computer and upload the PDF using the Field Map Layout screen, the plugin overwrites any PDF with the same name and there is no need to remap the fields*.
*Even if you change the PDF name, there is no need to remap the fields, simply select the Form and Field Map Layout from the top dropdowns and change the PDF Form dropdown to the newly named form. The plugin will recognize the field names and map to the new PDF filename.
