We got the screenshot, but need we need to check the actual PDF form. Please provide the PDF.
Sounds like there may be duplicate fields sitting on top of the mapped fields, which could happen in Adobe Acrobat if you select all fields then move while Ctrl is depressed.

Our apologies, we should have mentioned in the first message that if you are using Formidable’s “separate values” option and you are not using the “label” (‘display value’ as referred by Formidable) as the PDF value, then there is no need to set the Format dropdown to “Show label for checkbox” in the Field Map Layout…
Changing the format setting to “(no formatting)” in the Field Map Layout corrected the issue, and now all works as expected.

The plugin is not compatible with LiveCycle forms, it must be a PDF Form.
Solution: Use Adobe Acrobat or PDFEscape.com to recreate the PDF.
If you absolutely need the LiveCycle form to work with the plugin, we could create a customized version for you. If so desired, please complete this form to receive an offer.

It sounds like all your PDF form’s checkboxes have the same name… To configure Formidable’s Checkbox fields for “Multi-Select” values…
. Enter a unique name for each PDF Checkbox.
. Change the “Value” of each PDF Checkbox to match one “Value” of the Formidable Checkbox.If you have setup like the above directions, please email a link to the Formidable form and a copy of the pdf for further assistance to support AT formidablepro2pdf DOT com.
No worries!!! This is a good support thread because it really does look like a read-only or disabled field. It’s a lot like a Windows Username & Password login box when there is no password set, most people will never try “no password” and be locked out forever…
Thanks for using the plugin, do not spend a lot of time trying to figure something out if something doesn’t seem to be working correctly, check this forum and if you don’t see it, then don’t hesitate to ask us, as we are truly at your service!

It was not marked as read-only, the grayed out text is the ‘default filename’ you can just type over it. 😉

Remove the “.” (period) from the PDF Form filename, then reupload and change the PDF Form in the Field Map Layout.

Do you want us to provide you with a price quote to customize the plugin or can you use the emailed attachment?
This plugin will send merged PDF’s in emails without saving the data, however, there’s just no way to download data – at any time – if no data has been saved… Where would the data come from?
Here are your options;
. Use the “send PDF as an email attachment” to sent to a dedicated email address that you can use to ‘download’ the pdf at any time or email to the form’s user.. Request a customization quote for the plugin to provide you with a timed (temporary) download option.

This is an odd one… we have no prior occurrence of the issue you’ve described with the paid version of the plugin.
Please try the following;
. Reboot your computer and then open one browser window and clear the browser’s cache; if no change then;
. Create a duplicate Field Map Layout using the “Duplicate this field map” located at the bottom of the field map layout and see if the new copy works as expected.Let us know if the above solves the issue, as we remain at your service.
After testing yesterday and today, all worked as expected. We were unable to reproduce the issue. Please clear your browser’s history/cache and if you are still experiencing the issue please try another browser and let us know if it works while we remain at your service.
Paste the shortcode in the form’s ‘On Submit’ message box to generate the download button. See this part of the demo video for how to change the shortcode to link to the current dataset…
*Note: You can also paste the shortcode in the email body to create a link to download the merged PDF.
Yes, you will be able to have the data laid on top (technically…populate pre-made PDF form fields with mapped data from Formidable).

Yes, the plugin works with Formidable’s email notifications by allowing you to select which email notifications receives all Field Map Layouts associated with the submitting form that have the “Attach file to Email notifications” AND the specific email notification selected in the Field Map Layout designer.

Yes, pdf’s may be attached to any Formidable Email Action and pdfs may be accessed from the plugin’s Field Map Layout page by WP admin users.
Editing the data however, would need to be done from the Formidable form, which is possible to do using these instructions from our friends at Formidable…

There is no workaround to installing PDFtk on the server – with associated permissions – it is required by the plugin, however, the internet version does not require PDFtk be installed on your server, as our server will process the merge, only the intranet versions require PDFtk.
