    When I created the document I named all of the field. “Initialsbox” is a field that I named that needs initials and is a name on the form.

    The PDF’s are still coming up empty except for one field. Please check again and try yourself.

    form was submitted.. I forgot include my activation key but you can see that I’m a paid member when you login. thanks for the help..

    Also I should have said the plugin “simply doesn’t work for me” no pun intended.

    I thought I might add that I have PDF form with 18 fields. They consist of 2 signature fields that share the same value , 11 Initials fields that share the same value, 2 date fields that share the same value, 2 agent ID fields that share the same value, 2 name fields that share the same value.

    I created a formidable form with 4 fields: Name field, date field (current date default value), an Initials signature field, and signature field.

    I mapped the 5 formidable field to the 18 pdf fields. Maybe there’s an issue with using the same field more than once.

    this plugin simply does not work. I have watched you short demo video and followed the instructions but still end up with a mostly blank form. the only field that populates is one signature field 21 fields.
